Transport companies involved in transportation are obliged to secure them. Therefore, drivers who transport valuable cargo and/or passengers must undergo a medical examination before each trip. Its main purpose is to detect the presence of intoxication/hangover/other altered health conditions and thereby prevent the risk of an accident. The log of pre-trip medical examination of drivers (you will find a sample of 2018 in this article) is a document that contains the conclusion of a medical professional regarding the driver’s health condition.
Official transport organizations are obliged to monitor the safety of transportation. To protect against risks, drivers are required to undergo a medical examination at all times. Not only the safety of the cargo, but also the lives of people depends on this.
The driver who sets off on the trip must be healthy and without signs of alcohol and/or drug intoxication.
The official document where information about the driver is entered is the logbook for registering pre-trip pre-shift medical examinations. For 2018, the features of its maintenance are as follows:

Since the register in question does not have a mandatory form, the law also does not say how to fill out the driver’s pre-trip inspection log in 2018. At the same time, we can recommend a variant of the magazine form that has been in force since the times of the USSR. It looks like this (taking into account current realities):

The main purpose of the inspection is to determine the driver’s readiness to travel, assess his condition and possible risks. A separate room is allocated for the examination, or employees of the organization go independently to a medical facility.
If a driver has hypertension, their blood pressure is noted for 10 days to determine the individual acceptable level. If all indicators are normal, there are no signs of intoxication or hangover, an admission stamp is given. If there are signs that the driver cannot go on a flight, this is indicated in the log, and the information is conveyed to management.
In Russia, the procedure for carrying out pre-trip examinations of drivers is accompanied by the filling out of a number of documents by medical workers. One of them is a log of pre-trip medical examination of drivers (a sample from 2017 is given below). This document accumulates information on the results of pre-trip inspections. The form of such a journal is not established by law, but there are several regulatory rules regarding its maintenance. They, in particular, regulate the types of media, the list of mandatory information reflected in the journal, and the procedure for their certification. Otherwise, the employer is guided by its own interests to develop and approve the appropriate form of the journal.

The work of drivers of buses, trucks and other vehicles should be accompanied by increased attention to their health. Current legislation requires that the management of transport companies ensure that drivers undergo regular medical examinations in order to detect alcohol or drug intoxication, high blood pressure, as well as various serious health problems. The “Log of pre-trip medical examination of drivers” is an official document in which information about the drivers and the medical examinations performed is entered. An authorized medical officer analyzes the information contained in the log and, based on it, draws conclusions about the reasons for removing drivers from work. Incidents involving alcohol and drug use should be reported to the organization's management.
There is no unified form of the document approved at the legislative level. However, there is a form recommended for use, approved by order of the USSR Ministry of Health dated September 29, 1989 No. 555. It is this form that is most often used in organizations.
The journal can be kept not only in paper, but also in electronic form. But to do this, the employer will have to take care of obtaining an electronic digital signature to protect information stored electronically.
If the document is kept in paper form, it must be bound and stamped. Each page must be numbered.
The results of the examination, recorded in the log, must clearly determine whether the driver has symptoms that do not allow him to go on a flight, or whether he is absolutely healthy.
A note in the log about whether the driver is cleared for the flight or not should not be the main purpose of the medical examination. Ordinary formality is not acceptable here. Danger on the road can arise at any time, especially when it comes to passenger transportation. Regular medical examinations will help identify the first signs of the development of dangerous diseases that can cause accidents during a trip.
The head of the transport department is responsible for maintaining the log. He must ensure that entries in the document are made regularly, in full and in strictly chronological order.
Entries in the journal are made by a medical worker (either full-time or working in a medical institution involved in conducting examinations). His responsibilities also include pre-trip and post-trip medical examination of drivers. If the results of the examination allow the driver to be sent on a flight, the doctor puts a stamp on the waybill and makes a note in the log. The conclusion is given to the driver along with the waybill.
Despite the fact that the document does not have an approved form, there is a list of information that must be contained in it. They are listed in paragraph 14 of the Appendix to the order of the Ministry of Health of the Russian Federation No. 835n.

Employers have the right to independently develop the form of the magazine. The main thing is that its content does not contradict the requirements of current legislation.
The absence of a “Log of pre-trip medical examination of drivers” in an organization is a violation that will entail administrative liability. The presence of such a log is checked by inspectors from Rostrudinspektsii and Rosavtonadzor. If they find violations of the procedure for conducting medical examinations, fines will be issued to the organization and officials (Part 3 of Article 5.27.1, Article 12.31.1 of the Code of Administrative Offenses of the Russian Federation). Their size is 30,000-130,000 rubles. for companies, individual entrepreneurs and officials 5,000-25,000 rubles.
